Primary Care Manager

About the Role
We are seeking an experienced and skilled Primary Care Manager to join our team at Southport and Formby Health (GP Federation). As a key member of our leadership team, you will be responsible for overseeing the delivery of high-quality primary care services to our patients.
The successful candidate will have a strong background in primary care management, with experience of managing teams and delivering results-driven projects. You will work closely with our clinical leads and staff to ensure that our services meet the needs of our patients, and that we are providing the best possible care.
Key Responsibilities
- Manage the day-to-day running of our primary care services, including Enhanced Health in Care Homes, Digital Clinical Hub, PCN Cancer Services, and the PCN Pharmacy Team.
- Collate and prepare performance reports, KPI data, and outcomes to inform decision-making and drive service improvement.
- Develop and implement standard operating procedures to ensure compliance with governance requirements and professional standards.
- Build relationships with external partners and stakeholders to promote our services and identify opportunities for collaboration and growth.
