About the Role:

Key Responsibilities:

  • Provide expert palliative care support and advice to patients, families, and care professionals through a 24-hour telephone support line.
  • Work collaboratively with Nurses and Healthcare Assistants to deliver individualized care plans and ensure seamless patient care.
  • Lead and empower Healthcare Assistants, providing guidance and support to ensure the highest standards of care.
  • Contribute to the development and implementation of palliative care policies and procedures.
  • Participate in educational programs and training to enhance knowledge and skills in palliative care.

About You:

  • NMC registered with post-registration experience in palliative and end-of-life care.
  • Ability to work flexibly, positively, and as part of a wider team.
  • Excellent communication and interpersonal skills.
  • Leadership and empowerment skills to support and develop Healthcare Assistants.
  • Knowledge of SystemOne and the independent sector is advantageous.
