Job DescriptionThis role involves delivering immediate and continuing nursing care to patients within their own homes or clinic setting. You will be responsible for assessing, planning, implementing, and evaluating evidence-based care for patients. Additionally, you will provide ongoing nursing care and support to patients and carers, promoting the role of the nurse and clinical excellence.
You will network with other professionals to coordinate a multi-disciplinary approach to care provision. You will be responsible for multi-agency care documentation in line with current practice and ensure that patients can make an informed choice about their current and future care needs.
The successful candidate will have experience in community nursing and be able to demonstrate excellent communication and interpersonal skills. They will also have a full driving license and access to a car.
In return, we offer a competitive salary of £29,970-£36,483 per annum, pro rata, plus benefits and opportunities for professional development.
