Patient Pathway Coordinator

As a key member of our team, you will play a vital role in ensuring the smooth delivery of patient care and administrative services at Oxford University Hospitals NHS Foundation Trust. Your primary responsibility will be to provide comprehensive administrative support to our clinical teams, ensuring that patients receive the highest quality care and service.
Main Duties- Provide primary administrative support to named Consultants, clinical staff, and patients under their care, including diary administration, correspondence support, and administrative duties as required.
- Support patients under the Clinicians' care by administering relevant diagnostic investigations and expediting them in line with the week diagnostic pathway and week Referral to Treatment (RTT) pathway.
- Review clinical correspondence to inform referrers, patients, and other relevant parties of patient pathway progress. Produce correspondence that is of high quality and conforms to the national target of working days from the point of clinical delivery/decision.
- Assist the Patient Pathway Team Leader and the Patient Pathway Manager with the management of the Diagnostic Patient Tracking List (PTL).
- Liaise with Inpatient Waiting List, Outpatient Booking, and Cancer Access Teams to monitor the week RTT pathway and Cancer Access Standards. Scheduling in-patient encounters and out-patient clinic appointments as necessary.
- Work with the Service Manager and all staff responsible for monitoring the week pathway (RTT) and Cancer targets for patients awaiting treatment to ensure compliance, intervening where necessary to expedite appointments/surgery to avoid breaches.
- Act on queries from referrers, patients, and other relevant parties in liaison with clinical staff. Take relevant action to provide reassurance and support to patients and their families.
