Clinical Programs Manager

About the Role:
The Manager, Clinical Programs will be responsible for driving the development of DaVita's integrated care strategy and preparing the organization for a shift to value-based care.
In this role, you will work on a team of highly motivated individuals with consulting, process engineering, change management, and/or clinical experience.
The Manager will collaborate with Kidney Care and IKC (Integrated Kidney Care) leadership to develop and implement integrated care programs.
Key Responsibilities:
- Evaluate effectiveness and scalability of field initiatives and prepare markets for the launch of new integrated care programs.
- Partner and collaborate with key teams across the Village to drive this strategy to fruition.
- Determine how to best organize our frontline and field leadership teams to best deliver integrated care for ESKD patients.
- Collaborate with key stakeholders across the Village to design scalable solutions for our teams and work to build consensus around recommendations.
- Evaluate performance of current initiatives and complete analysis to understand best opportunities to improve clinical outcomes and reduce total cost of care.

The Clinical Transformation team is responsible for driving the development of DaVita's integrated care strategy and preparing the organization for a shift to value-based care.
As a member of this team, you will work closely with Kidney Care and IKC (Integrated Kidney Care) leadership to develop and implement integrated care programs.
