About the Role

We are seeking an experienced Mental Health Team Lead to join our Louth Crisis Resolution & Home Treatment Team. The successful candidate will be responsible for coordinating care needs, assessment, planning, and implementation of evidence-based care to a defined group of service users.

Key Responsibilities:
  • To provide high-quality clinical care ensuring safe and effective assessment, treatment, and comprehensive discharge planning using highly developed clinical reasoning skills.
  • To ensure the clinical caseload and clinical practice meet the highest standards of clinical care.
  • To develop and maintain accurate and timely clinical records and risk assessments.

Salary: £50,000 - £60,000 per annum

Location: Lincolnshire Partnership NHS Foundation Trust, Louth, LN11 0LF

Job Type: Full-time

Experience: 2+ years in mental health care with experience of working with adults with mental health needs.
