Key Responsibilities
- Work creatively with individuals, local communities, and our partners to empower them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.
- Establish professional, effective working relationships with a range of partners/colleagues, adults, families, and their carers.
- Provide high-quality social work activities, including case management, assessment, and intervention.
- Work collaboratively with other teams and agencies to ensure a seamless service delivery.
Requirements
- Qualified social worker with registration with Social Work England.
- Strong value base that informs your work with adults.
