About the Role

We are seeking an enthusiastic and caring Assertive Outreach Support Worker to join our team in Surrey. As a key member of our Liaison and Diversion Service, you will work alongside colleagues to provide person-centred and community-based support and interventions, aiming to improve the health and social care outcomes for individuals with the ultimate goal of reducing offending.

Key Responsibilities
  • Provide assessments to identify vulnerabilities such as mental health concerns, physical health concerns, learning disabilities, substance misuse concerns, housing, finance concerns, and other vulnerabilities.
  • Offer and provide support for the identified vulnerabilities, which can involve signposting, referrals to appropriate support services, assistance with making and attending appointments based on their needs, collaborative working with other services and organisations, and providing wellbeing support in a person-centred holistic way.
  • Work collaboratively within a multidisciplinary team to provide excellent person-centred care, support, and planning.
  • Assess and manage risk and safeguarding, and have a working knowledge of assertive outreach work, policies, and procedures supporting these.
  • Have good communication skills, both written and verbal, and be proficient in Microsoft Word, Outlook, and Excel, with previous experience of an electronic patient record system preferable.
Requirements
  • Enthusiastic, with a drive to provide excellent person-centred care, support, and planning.
  • Willing to learn, and seek to improve quality of care.
  • Previous community experience working with individuals relating to mental health, learning difficulties, drug and alcohol addiction, homelessness, etc.
  • Knowledge of the criminal justice system and/or Health and Social Care is desirable.
  • Ability to work collaboratively within a multidisciplinary team.
  • Working knowledge of assertive outreach work, assessing and managing risk and safeguarding, policies, and procedures supporting these.
  • Good communication skills, both written and verbal.
  • Proficient in Microsoft Word, Outlook, and Excel, with previous experience of an electronic patient record system preferable.
