About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our Community First Team in Derby City. As a key member of our team, you will play a vital role in providing high-quality care and support to patients in their own homes.

Key Responsibilities

  • Provide personal care and support to patients, promoting independence and dignity.
  • Work collaboratively with clinicians and other healthcare professionals to deliver effective care plans.
  • Contribute to the development of a range of knowledge and competencies to support clinicians.
  • Assist with the delivery of agreed programmes of treatment and care.

Requirements

  • GCSE Maths & English (C/4 or above) or equivalent.
  • Previous experience in a health & social care setting (min. 1 year).
  • Excellent communication and interpersonal skills.

  • 27 days annual leave/year plus bank holidays, increasing to 29 days after 5 years & 33 days after 10 years' service.
