About the Role

We are seeking a highly skilled and motivated Care Coordinator to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a Care Coordinator, you will play a vital role in supporting working-age adults with moderate to severe affective and psychotic disorders.

Key Responsibilities
  • Undertake assessments of individuals and their families/carers accessing the service with complex health presentations.
  • Develop and implement care plans alongside service users and their families/carers.
  • Offer specialist professional assessment and advice where required.
  • Assess and manage ongoing risks, ensuring collaboration with service users and carers.
  • Regularly review risk factors and make changes to their management as necessary.
  • Work using assertive outreach approaches to engage service users and their families.
  • Contribute to the maintenance and development of the Trust's nursing strategy.
  • Ensure clinical practice is evidence-based and consistent with relevant NICE guidelines and Trust policies and procedures.
  • Provide supervision of junior staff and trainees where appropriate.
  • Act within the professional code of conduct at all times.
Requirements
  • Registered Nurse with current registration (mental health/LD/Adult), appropriate to the job role.
  • Registered Social Worker or Registered Occupational Therapist.
  • Completion of Mentorship Course/ENB equivalent, or intention to complete.
  • Evidence of continuous professional development.
  • Significant post-qualifying experience in working with those presenting with serious mental illness.
  • Experience of working alongside people with complex health problems in the designated care group.
  • Experience of supervising staff.
  • Experience of managing a caseload.
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems.
  • Experience of conducting clinical assessments, including risk assessments.
  • Experience and knowledge of dual diagnosis/substance misuse.
  • Previous experience of working in the community.
Desirable
  • Lived experience of mental health issues.
