We are seeking an Adult Complex Healthcare Professional to join our team at Harmony Healthcare.

Key Responsibilities:

  • Develop and maintain strong relationships with clients, families, and other healthcare professionals to ensure seamless care coordination.
  • Stay up-to-date with relevant industry developments, best practices, and research to inform care decisions.
Requirements:
  • Proven experience in caring for adults with complex needs, preferably in community-based settings.
  • Able to work flexibly, including evenings, weekends, and bank holidays, to meet client needs.
  • Excellent communication and interpersonal skills, with a patient-centered approach.
