Clinical Care Coordinator

We are seeking a skilled Clinical Care Coordinator to provide patient care and collaborate with other healthcare professionals to ensure the best outcomes for our patients.
Responsibilities:
- Assess and monitor patient health, administering medications and treatments as required.

Requirements:
- Current licensure as a Registered Nurse (RN) in good standing with the relevant nursing board.
