Location: Lawrence Hill Health Centre, Bristol

Salary: £25,000 - £28,000 per annum (dependent on experience)

Job Description:

We are seeking an experienced Patient Support Coordinator to join our team. The successful candidate will work closely with our GPs and other primary care professionals to identify and manage a caseload of high-impact frequent service users. This role involves proactively identifying vulnerable patients, reviewing their needs, and helping them access the services and support they require to understand and manage their own health and wellbeing.

The ideal candidate will have excellent communication and interpersonal skills, be able to work independently and as part of a team, and have a strong ability to problem-solve. They will also be familiar with data collection and using tools to measure the impact of services.

Key Responsibilities:

  • Proactively identify and work with a cohort of patients to support their personalised care requirements
  • Bring together all of a person's identified care and support needs and explore their options to meet these into a single personalised care and support plan (PCSP)
  • Help people to manage their needs, answering their queries and supporting them to make appointments
  • Identify barriers to or issues preventing greater engagement and/or uptake by patients actively participate in developing solutions for patients and service providers
  • Raise awareness of shared decision making and decision support tools and assist people to be more prepared to have a shared decision-making conversation

Person Specification:

Essential:

  • GCSE grade A-C English and Maths
  • Excellent telephone and interpersonal skills
  • IT literate
  • Ability to work to time and be flexible in approaches to different situations
  • Ability to work independently on their own or as a part of a team
  • Able to show initiative and problem solve

Desirable:

  • NVQ level 3 in adult care
  • 2 Day Personalised Care Institute accredited Care Co-ordinator training
