Key Responsibilities
  • Provide high-quality person-centered support to enable and empower service users to develop the skills necessary for independent living within legislative, regulatory requirements and best practice.
  • Establish professional relationships with all service users, using appropriate and clear communication to promote independence through developing daily living skills and social engagement, ensuring that they maintain personal choice, privacy, and dignity.
  • Provide an intensive housing management service to enable service users to maintain their tenancy, stay safe, and understand their rights and obligations under their tenancy.
  • Assist service users in understanding and planning their support package, focusing on positive outcomes to maximize their potential and enable them to exercise choice and control.
  • Provide advocacy to support service users and their families when required, to maximize their independence and personal potential.
  • Liaise with relevant external agencies to ensure the maximum support is delivered to enable the service user access to and maintain a level of integration within the wider community.
  • Manage the implementation of safeguarding principles and responsibilities to ensure the safety of service users and to avoid potential risks.
  • Promote self-practices and undertake risk assessments within legislative frameworks and business protocols, escalating issues as appropriate.

Requirements
