Complex Needs Practitioner
About the Role:We are seeking a skilled and compassionate Complex Needs Practitioner to join our team. As a key member of our Transitions team, you will work with young people aged 17-25 who are affected by substance misuse, including those impacted by their parent's substance misuse.
Your primary focus will be to engage young people in a variety of interventions, using a young person-centred approach to empower them to reduce risk, increase resilience, and achieve positive outcomes.
Key Responsibilities:- Work with young people and their families to address holistic support needs across all aspects of their wellbeing.
- Develop and implement individualised support plans to address substance misuse, mental health, and other related issues.
- Collaborate with multi-agency partners, including health, social care, education, and criminal justice, to ensure a comprehensive approach to support.
- Provide guidance and support to young people to help them navigate complex systems and access necessary resources.
- Contribute to the development and delivery of training and education programs for young people and their families.
We are looking for an energetic and confident self-starter with proven experience in health and social care, particularly working with young people and supporting them through mental ill health.
You will have excellent communication and interpersonal skills, with the ability to build strong relationships with young people, families, and multi-agency partners.
