About the Role

This position involves working as part of our Permanence Support Team in Woodley, Reading. Your primary focus will be providing post-order support to adopters, special guardians, children, and birth families.

Key Responsibilities:
  1. Support Needs Assessment: Conduct detailed assessments of adopters, special guardians, and birth families to identify their unique support requirements.
  2. Ongoing Support: Offer regular guidance and intervention to ensure stable placements and optimal child outcomes for adoptive or special guardian families.
  3. Birth Family Engagement: Collaborate with birth families to ensure they receive necessary support and information about adoption and special guardianship, facilitating their understanding of their ongoing role.
  4. Therapeutic Interventions: Provide therapeutic support to adopters, children, and families, promoting emotional and psychological wellbeing, and developing strategies for managing challenging behaviors and attachment issues.
  5. Multi-Agency Collaboration: Work closely with healthcare professionals, educational services, and legal representatives to guarantee comprehensive post-order support.
  6. Professional Development: Deliver training to adopters, special guardians, and other professionals on post-adoption matters and ensure access to relevant resources and services.
  7. Safeguarding: Prioritise children's safety and welfare in all aspects of the role, adhering to appropriate safeguarding procedures.
  8. Case Management: Maintain accurate, confidential case records and reports according to departmental policies, ensuring key information remains accessible.
About You

To excel in this role, you must be a qualified Social Worker with adoption or family support experience. Strong understanding of adopters', special guardians', and birth families' challenges is crucial, along with a commitment to enhancing the lives of children and families affected by adoption.
