In this role, you will be responsible for delivering high-quality nursing care to patients in their homes or community settings. You will work collaboratively with GPs, social care teams, and other healthcare professionals to ensure a seamless and effective care delivery process.

The successful candidate will possess:

  • A valid NMC registration
  • Experience in community nursing or a similar setting (desirable but not essential)
  • Excellent communication and organizational skills
  • A passion for delivering patient-centered care
  • A full UK driving license and access to a vehicle
