Cambridgeshire & Peterborough NHS Foundation Trust is seeking a skilled Healthcare Professional to join their team in supporting the uptake, training, and provision of annual health checks for individuals with learning disabilities.

The successful candidate will be at the forefront of improving the quality of annual health checks for the county's learning disability population. This role requires experience working with adults with learning disabilities, nursing assessment and intervention relevant to this population, and effective communication skills.

The key duties of this position include working under the guidance of the lead nurse to improve the uptake and quality of Annual Health Checks for the Learning-Disabled population across Peterborough. The professional will also participate in multi-professional/multi-agency working within the LDP/Peterborough Learning Disabilities team and with Key partners - particularly the GP surgeries within the C& PCCG.

To succeed in this role, you should have excellent organisational and time management skills, ability to reflect and critically appraise your own performance, and ability to prioritise your work effectively. Experience in interagency working and Systmone is desirable.

Working as part of an MDT and participating in difficult conversations is essential for this role. You should have knowledge of the Mental Health Act and Mental Capacity Act, as well as awareness of current trends, legislation, and guidance in support for people with learning disabilities and their carers.

Key Responsibilities:

  • Improve the uptake and quality of Annual Health Checks for the Learning-Disabled population across Peterborough.
  • Participate in multi-professional/multi-agency working within the LDP/Peterborough Learning Disabilities team and with Key partners.
  • Provide ongoing support and liaison to GP practices within the clinical area.
  • Devise a questionnaire to measure the quality of AHC and lead on evaluation of results.

Salary: £30,000 - £40,000 per annum (dependent on experience)

Location: Peterborough
