Care Coordinator for Children with Complex Needs

PP Associates Ltd is seeking an experienced care coordinator to join their team. This role involves working closely with children who have complex needs, including learning disabilities and life-limiting conditions.
The ideal candidate will have a strong background in assessment and adaptation, with experience of working within Local Authorities.
Awareness of policy and guidance related to Disabled Facilities Grants (DFGs) is essential.
Key Responsibilities- To assess the needs of children with complex needs and develop suitable plans for adaptations
- To work collaboratively with Local Authorities to ensure successful implementation of adaptations
- To stay up-to-date with relevant policies and guidelines, ensuring best practice in care coordination
We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience.
Requirements- Experience of working with children with learning disabilities and complex health needs
- Strong assessment and adaptation skills
- Awareness of DFG policy and guidance
