Job Description

We are seeking an experienced Senior Practitioner to join our team in the Children with Disabilities Team. This full-time role will be based in Kingston and involve working closely with children, young people, and their families to develop individualised plans and ensure effective outcomes.

About the Role

You will be responsible for assessing children's needs, developing care plans, and delivering interventions to achieve positive outcomes. Your expertise in care coordination, risk management, and communication will be essential in building strong relationships with children, young people, and their families.

Key Responsibilities
  • Assessment and Care Planning: Develop comprehensive care plans that address the unique needs of each child, incorporating input from families and other professionals.
  • Care Coordination: Chair meetings to review progress, identify areas for improvement, and make informed decisions about future plans.
  • Intervention and Support: Deliver evidence-based interventions to engage children, young people, and their families, promoting positive outcomes and empowering them to make informed decisions.
Requirements and Qualifications

To succeed in this role, you will need:

  • Experience: A minimum of 2 years' experience in a similar role, preferably within the children's services sector.
  • Expertise: Proven expertise in assessment, care planning, and intervention strategies for children with disabilities.
  • Qualifications: Relevant qualifications, such as a degree in social work or a related field, and/or equivalent experience.

This role offers a competitive salary of £49,083 per annum, along with a range of benefits, including:
