Old Road Medical Practice is seeking a skilled Practice Nurse to contribute to our multidisciplinary team. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and promoting health and well-being in our community.
Main Responsibilities
- Conduct comprehensive health assessments, administer medications and treatments, and perform diagnostic tests to ensure accurate diagnoses and effective patient care.
- Develop and implement individualized health plans to address patients' complex needs, including long-term conditions, and provide education and support to promote healthy behaviors.
- Collaborate with healthcare professionals to coordinate patient care, share knowledge, and improve health outcomes.
- Manage and treat long-term conditions, adhering to national and local policies, and practice protocols.
- Review and optimize medication regimens to ensure therapeutic effectiveness and patient adherence.
- Respond to urgent or emergency situations, prioritizing patient needs and providing effective care.
- Support patients with sexual health and contraception needs, and provide cervical cytology consultations.
- Implement and participate in vaccination and immunization programs for adults and children, including travel vaccinations.
- Provide wound care services, including dressing and wound management.
- Conduct hypertension reviews and manage patients with asthma, COPD, and diabetes.
- Perform new patient and health checks, and provide preventive care services.
