About the Role

We are seeking a highly skilled and compassionate individual to join our team as a Population Health Manager. This is an exciting opportunity to work with our Primary Care Network (PCN) to support the delivery of our Population Health Management Programme.

Key Responsibilities
  • Support the ongoing identification, engagement, and case management of patients with mild to moderate frailty.
  • Work closely with an Advanced Nurse Practitioner to triage patients into a pathway and provide regular check-ins.
  • Coordinate patient care plans and ensure patients receive the necessary support and services.
  • Develop effective working relationships with multiple professionals across the sectors to ensure seamless care coordination.
  • Support the PCN in implementing the programme by working within governance structures and providing feedback.
Requirements
  • Relevant health and social care experience at AfC Band 3 or 4 or equivalent.
  • Experience of administrative duties and working in Primary Care.
  • Ability to work effectively as a member of a team and develop effective working relationships.
