Job Title: Advanced Nurse Practitioner - Population Health Management

We are seeking an experienced Advanced Nurse Practitioner to join our team in Oldham South PCN. The successful candidate will be responsible for leading the assessment of patients over 65 with mild to moderate frailty and developing person-centred shared care plans in collaboration with patients and healthcare professionals.

Main Responsibilities:
  • Conduct comprehensive geriatric assessments to identify patients with complex needs and produce accurate records of patient consultations.
  • Refer patients to health, care, and community-based services as needed for continuation of care.
  • Develop and implement person-centred shared care plans for patients in collaboration with patients and healthcare professionals.
  • Support the leadership of the PHM care coordinator to identify and engage patients for early intervention and maintain good health.
  • Work within the PCN team and across an integrated neighbourhood team to create care plans.

Person Specification:
  • Advanced clinical practice skills
  • Significant post-registration experience
  • Recent primary and community nursing experience
  • Nurse-led triage
  • Management of patients with long-term conditions and complex needs
  • Clinical examination skills
  • Experience of administrative duties
  • Working in a multi-disciplinary setting
  • Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
  • Experience in use of databases
