About the Role:

We are seeking a highly skilled and compassionate Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a vital role in supporting our customers who require mental health support to develop their skills and live more independently in the community.

Key Responsibilities:

  • Creating personalized support plans with customers to help them achieve their goals and aspirations.
  • Providing emotional support and guidance to customers who are pregnant, have just had a baby, or require social support while receiving support from NHS Crisis teams.
  • Helping customers access housing, financial support, and other essential services.
  • Conducting risk assessments, support planning, goal setting, and regular reviews to ensure customers receive the best possible support.

Requirements:

  • Passion for supporting customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centered support plans and supporting colleagues in working to the plans.
  • Ability to work independently, remain calm under pressure, and have a resilient approach.
