Job Title: Community Health Nurse - Band 6

We are seeking an experienced Community Health Nurse to join our team at Great Western Hospitals NHS Foundation Trust. As a Community Health Nurse, you will be responsible for providing high-quality care to patients in their own homes and community settings.

Main Responsibilities:

  • Provide effective caseload management and care to patients over the age of 18 requiring nursing care in their own homes and other community settings.
  • Work autonomously and independently, providing holistic care, assessment, and treatment to patients with complex care needs or long-term conditions.
  • Collaborate with the multidisciplinary team to ensure seamless patient care journeys.
