Key Responsibilities:
  • Coordinate quality and safe discharges in a timely manner
  • Support the care homes and external discharge teams with residents who require additional, individual planning
  • Provide a person-centred, quality and effective service
Requirements:
  • Previous experience in a clinical environment with knowledge/experience of continued care planning and delivery desirable
  • Confident in liaising with management and external professionals across multiple disciplines and able to maintain effective professional relationships
  • Good time management and organisation skills
  • Minimum requirement of NVQ Level 3 in Health & Social Care unless expansive experience can be demonstrated
