Job Summary

We are looking for a highly skilled Advanced Clinical Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. The successful candidate will have extensive experience in assessing complex patient needs, delivering holistic care, and exercising advanced clinical expertise.

Key Responsibilities

  • Assess and manage patients with long-term illnesses and older adults
  • Deliver medical care for patients from sub-acute care to general rehabilitation
  • Exercise advanced clinical assessment and management skills
  • Lead on improvements to standards of care through supervision of practice, clinical audit, evidence-based practice, and facilitation of learning
  • Develop and maintain effective working relationships with multidisciplinary teams
