Job Overview

We are seeking a skilled Nurse-Led Care Coordinator to join our team at Center Healthcare.

  • Lead a team of care workers to provide high-quality, nurse-led care to clients with complex health needs.
  • Develop and implement personalized care plans that meet the unique needs of each client.
  • Collaborate with healthcare professionals to ensure seamless transitions of care.
  • Conduct regular assessments and reviews to ensure optimal client outcomes.
  • Minimum 2 years experience in a care setting, particularly with complex care needs.
  • Strong leadership and coordination skills.
  • Ability to work independently and as part of a team.
  • Enhanced DBS check required.
  • Competitive Salary: £40,000 - £50,000 per annum, with enhancements for unsociable hours.
