Job Title: Adult Nurse Manager

About the Role:

We are seeking an experienced Adult Nurse Manager to join our team at Leaders in Care. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans for each individual.

Key Responsibilities:

- Design and deliver tailored care plans specific to an individual client's needs
- Effectively manage the multidisciplinary team through strong communication and teamwork
- Provide ongoing monitoring, support, and assessment of client's physical and physiological needs within your caseload
- Deliver nursing interventions, including the management of medicine delivery
- Build and maintain sophisticated relationships with your client's family

Requirements:

- RGN with an active NMC PIN
- Supportive management style that can bring out the best in others
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT
- Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs

Benefits:

- Up to £45,000 per annum DOE
- 1 day working from home per week / 4 days in the community
- Monday to Friday (9-5) hours
