We are seeking a dedicated and compassionate Rapid Response Support Specialist to join our team at Superior Healthcare. As a specialist in complex care, you will provide high-quality support to clients with long-term health conditions, working closely with our nurse-led team to deliver exceptional care.
Key Responsibilities:
- Provide personalized support to clients with complex care needs, including tracheostomy, ventilator care, epilepsy, and PEG management.
- Work collaboratively with our nurse-led team to develop and implement individualized care plans.
- Conduct regular assessments and reviews to ensure the highest level of care and support.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in ongoing training and professional development to enhance your skills and knowledge.
Requirements:
- A minimum of 12 months experience in care, with a proven track record of supporting adults and children with complex care needs.
- Right to work in the UK and a clean UK driving license.
