Key Responsibilities:

  • Provide personalized support to clients with complex care needs, including tracheostomy, ventilator care, epilepsy, and PEG management.
  • Work collaboratively with our nurse-led team to develop and implement individualized care plans.
  • Conduct regular assessments and reviews to ensure the highest level of care and support.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing training and professional development to enhance your skills and knowledge.

Requirements:

  • A minimum of 12 months experience in care, with a proven track record of supporting adults and children with complex care needs.
  • Right to work in the UK and a clean UK driving license.
