About the RoleThe successful candidate will be responsible for providing face-to-face support to adult clients and their families, creating personalised care plans, and working closely with a core team of carers to train and support them with their clinical skills and activities.
Key Responsibilities- Design and deliver tailored care plans specific to individual client needs
- Effectively manage the multidisciplinary team through strong communication and teamwork
- Provide ongoing monitoring, support, and assessment of clients
- Develop and implement effective care strategies to meet client needs
- RGN with an active NMC PIN
- Supportive management style that brings out the best in others
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years
- Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT
- Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs
- Full UK Driving Licence desirable but not essential
