Nurse Practitioner Specialist

Salary Range: £55,000 - £65,000 per year
Job Description:This is an exciting opportunity to join our dynamic team as a Nurse Practitioner Specialist at Upwell Health Centre. As a highly skilled and autonomous practitioner, you will play a vital role in providing safe, holistic care to our patients.
Your key responsibilities will include:
- Working closely with the clinical team to develop and implement effective chronic disease management programs.
- Conducting regular reviews and assessments of patients with complex needs, including those with asthma, diabetes, and coronary heart disease.
- Maintaining accurate records and participating in clinical supervision to ensure high-quality patient care.
To succeed in this role, you will require a Master's degree in Advanced Practice, significant experience working in a similar capacity, and excellent analytical and problem-solving skills.
