Job Title: Experienced Care Coordinator

Location: Derbyshire area

About the Role:

We are seeking an experienced Care Coordinator to join our team in Derbyshire. As a Care Coordinator, you will play a crucial role in coordinating care services for our clients, ensuring their needs are met and exceeded.

Key Responsibilities:

  • To coordinate care services for clients, including assessing their needs, developing care plans, and monitoring progress
  • To communicate effectively with clients, families, and healthcare professionals to ensure seamless care delivery
  • To work closely with our care team to ensure high-quality care is delivered to our clients
  • To maintain accurate records and reports, ensuring compliance with regulatory requirements

Salary: £35,000 - £45,000 per annum, depending on experience

Requirements:

  • A car driver's license is essential
  • Highly passionate individual with a strong commitment to delivering exceptional care
  • Previous experience working in adult domiciliary homecare is essential

About Us:

We deliver private homecare as well as NHS and Live-in Care packages. Our team of 5 includes Care Coordinators, Field Care Supervisors, and excellent Care Assistants.
