Job Opportunity: Heart Health Service Coordinator

Are you passionate about delivering high-quality patient care and improving health outcomes? Do you have experience as a nurse consultant in heart failure and a non-medical prescriber? We are seeking a skilled Heart Health Service Coordinator to join our team at the Transformation Unit.

This is an exciting opportunity to work in a fast-paced environment and make a real difference in the lives of our patients. As a key member of our Integrated Heart Failure Service, you will be responsible for developing and implementing a cohesive management programme for patients with heart failure, working across acute, community, and virtual ward settings to prevent unnecessary admissions to hospital and improve heart failure management and control.

You will need to be a NMC registered nurse with physical health assessment skills and experience as a non-medical prescriber. You will also be responsible for providing training and mentorship to colleagues, contributing to the development of policy and services to reflect the needs of the target population, and ensuring that patients receive the most effective care possible by assessing their conditions and considering a range of options when delivering complex and highly complex care.
