Learning Disability Support Specialist

We are seeking a highly motivated and dedicated individual to join our integrated Health and Social Care team in North Norfolk. As a Learning Disability Support Specialist, you will play a vital role in supporting adults with a Learning Disability to live independent and fulfilling lives.
Key Responsibilities- Work collaboratively with a multi-disciplinary team to provide specialist support to adults with a Learning Disability
- Develop and implement personalized support plans to promote independence and well-being
- Build strong relationships with service users, their families, and other stakeholders to ensure effective multi-agency working
- Contribute to the development and delivery of training programs to enhance the skills and knowledge of colleagues
