Job Overview:
Hoop Social Work is seeking a qualified Social Worker to join their Complex Intervention and Treatment Team (CITT) in Bath. The ideal candidate will have significant experience working with older adults, particularly those with mental health needs such as Alzheimer's and dementia.

About the Role:
As a member of the CITT team, you will work closely with colleagues to provide high-quality care and support to individuals requiring complex interventions. Your primary focus will be on delivering evidence-based treatments and developing personalized care plans that cater to the unique needs of each client.

Key Responsibilities:
• Work collaboratively with multi-professional teams to develop and implement effective care strategies
• Conduct thorough assessments to identify clients' needs and goals
• Develop and maintain strong relationships with clients, families, and other stakeholders
• Stay up-to-date with relevant legislation, policies, and best practices in social care

Requirements:
• A minimum of 6 months' experience as a registered social worker in the UK
• A relevant Social Work qualification
• Registration with either Social Care Wales or Social Work England
• Enhanced DBS clearance and access to regular updates
• 3 years of professional references
• Valid driving license and access to a car for work-related travel
