Job Description



We are seeking an Advanced Community Physiotherapist Specialist to join our team. As a key member of our multidisciplinary community team, you will work closely with physiotherapists, occupational therapists, nurses, specialist paramedics, pharmacists, and other healthcare professionals to deliver expert interventions and rehabilitation to patients in their homes or community settings.



You will be responsible for providing comprehensive assessments, planning programs of care, and evaluating patient outcomes. Your expertise will be crucial in helping patients regain independence, preventing unnecessary hospitalizations, and supporting early discharge from hospital.



Required Skills and Qualifications



To succeed in this role, you must have:




  • A degree in physiotherapy or a related field
  • Registration with the Health and Care Professions Council (HCPC)
  • Experience working in a community or in-patient setting as a physiotherapist
  • Excellent assessment, planning, and communication skills
  • Able to work effectively in a multi-disciplinary team environment
