About the Role
We are seeking a skilled Community Case Manager to join our Southern Adult Community Healthcare Team. As a key member of our team, you will play a vital role in providing high-quality patient care and contributing to the development of our services.
Key Responsibilities
- Contribute to the development of case management programs for patients with complex needs.
- Assess and plan care for patients with long-term conditions, working closely with multidisciplinary teams.
- Implement evidence-based interventions to improve health outcomes for patients.
- Develop and maintain partnerships with community-based services to enhance patient care.
- Manage a caseload of patients, prioritizing their needs and coordinating care delivery.
Requirements
- Registered Allied Healthcare Professional or Registered Level 1 Nurse.
- Degree or Diploma in Health or Nursing Studies or related subject.
- Evidence of recent Continuous Professional Development.
- Relevant postgraduate clinical training in areas such as elderly care, long-term conditions, and rehabilitation.
