**Job Overview**

Care Outlook Ltd, an expanding leading home care provider in London and South East of England since 2005, is seeking a highly skilled Care Management Specialist to join our Hounslow branch.

Responsibilities
  • Roster and manage care calls using electronic systems, ensuring seamless service delivery.
  • Directly oversee the domiciliary care team, promoting staff development through regular supervisions, appraisals, and training sessions.
  • Create, monitor, and update customer care plans, ensuring adequate staffing rotations meet clients' needs and aspirations.
  • Guarantee care assessments, planning, and risk management adhere to best practices and client expectations.
  • Provide emergency care services as needed, arrange backup coverage when required, and contribute to all reasonable calls.
  • Ensure prompt responses to client inquiries, maintaining a respectful and courteous tone.
  • Collaborate with the Branch Manager to address quality standards, complaints, and feedback efficiently.
  • Participate in the paid on-call rota system.
  • Implement measures to maintain CQC compliance and strive for exceptional care standards.

Requirements
  • A commitment to enhancing the quality of life for vulnerable individuals.
  • Familiarity with CQC standards and compliance (essential).
  • Experience in managing care calls on electronic systems.
  • Previous experience in care coordination or at least 2 years in healthcare and social care (preferred).
  • A full driving license and access to a vehicle (preferred).
  • NVQ Level 3 in Health and Social Care (desired).
