Job Title: Healthcare Transition Coordinator

We are seeking an experienced Healthcare Transition Coordinator to join our team at the NHS. As a key member of our team, you will play a critical role in ensuring seamless transitions between healthcare settings.

In this role, you will be responsible for assessing patient needs, determining priority care, and processing referrals. You will also work closely with the senior clinical team to develop knowledge and skills in SystmOne ledger management.

The estimated salary for this position is £35,000 - £45,000 per annum, depending on experience.

Key Responsibilities
  • Assess and process incoming referrals, prioritizing patient needs
  • Collaborate with the senior clinical team to develop knowledge and skills in SystmOne ledger management
  • Maintain awareness of integrated services demand and capacity across the system
  • Escalate concerns when necessary, liaising with clinical teams in the community and integrated team
Requirements

  • Ability to work independently and as part of a team
