About the Role

Hunter AHP is seeking a skilled and dedicated Care Coordinator to join our Adult team in the North West region. As a key member of our multidisciplinary team, you will be responsible for coordinating, commissioning, and delivering individual packages of care for a caseload of service users within Community Mental Health Teams (CMHTs).

Key Responsibilities
  • Undertake initial assessments of new referrals to the service.
  • Coordinate and deliver individual packages of care for a caseload of service users.
  • Work collaboratively with multidisciplinary teams to provide high-quality patient care.
  • Support the duty system and contribute to the smooth operation of the team.
Requirements
  • Current and valid registration with the Nursing and Midwifery Council (NMC).
  • Proven experience within a Care Coordinator or similar role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Commitment to providing high-quality patient care.
