Main Duties of the JobYou will hold and manage a caseload of people and have Care Coordinating responsibilities under the Care Program Approach Guidelines. The team covers an ethnically and culturally diverse area, and you will have the opportunity to work with clients from a wide range of backgrounds and with a wide range of psychosocial needs.
You will work with clients to identify goals as part of overall care plans, and establish and evaluate appropriate treatment programs for service users within the team. The post holder will be expected to participate in providing quality first-line psychosocial assessments as part of GMMH's Functional CMHT's duty role and work proactively in forging professional working relationships with multi-agencies.

We are looking for a qualified Mental Health Practitioner with experience of working in a community mental health setting. The ideal candidate will have a professional qualification (RMN/OT/SW) and professional registration. Postgraduate qualifications in mentorship or NMP are desirable.
The successful candidate will have experience of using electronic recording systems and will be able to work proactively in forging professional working relationships with multi-agencies.
