An exciting opportunity has arisen to work within our Community Health & Justice Services.
We are looking for mental health practitioners within an interest in mental health of people in the criminal justice system. GMMH offers assessment and access to mental health care through the 5 borough-based teams across our footprint.
We are seeking enthusiastic practitioners to join our teams in Salford and Bolton.
We currently have 1 full time contract in Salford & a 30 hour contract in Bolton.
You will work with a multi-agency team delivering individualised care to people who come into or at risk of coming into contact with the criminal justice system.
The post holder/s will carry a caseload within which they will provide assessments, care formulations, clinical risk assessments. Additionally they will liaise with & on behalf of service users to enable timely access to health and social care as well as diversion away from the criminal justice system where appropriate. The post holder/s will be expected to work closely and in collaboration a range of care and criminal justice partners such as, housing officers, police officers/staff, probation officers, GP’s, community health services. This is a permanent contract post for a qualified mental health nurse or allied health professional at Band 6 level.
You would have the support of a Senior Clinical/ Management Team, as well as the skills and experience of a diverse Multi-disciplinary Team within the mental health services.
These posts are offered as a permanent opportunity for an experienced band 6 mental health practitioners to work within the Health and Justice Community Services. The roles will be based at April House in Manchester, St Helena Mill in Bolton, St James’s House in Salford and Stretford Police Station in Trafford. The role will involve the case management of service users in the community with mental health problems or neuro diversity difficulties who find themselves in contact with the criminal justice system.

Principal Duties and Responsibilities - Work closely with a high demand cohort of service users with mental health problems or learning disabilities.
- Provide high quality case management to service users with complex needs.
- Receive referrals and actively engage person/s in a dedicated time frame.
- Be able work in an assertive outreach based model.
- To participate in the development of service policy and procedures as well as ensuring that all policies and procedures are implemented and adhered to.
- Support the evaluation of practice and other aspects of service provision and implementing appropriate measures for improvement.
- Responsible for providing line management and clinical and managerial supervision to junior team members.
- Assess, plan, implement and evaluate programmes of support for service users.
- Be able to use GMMH systems of recording and communication.
- Refer individuals into services where a need has been identified and there is a service to support.
- To maintain clear lines of communication between services, with particular regard to risk assessment and risk management.
- Contribute to the development of standard operating procedures for the service.
- Be comfortable working and dealing with users and enquiries in a health and justice setting.
- Support service users to attend and engage in services which seek to reduce criminal behaviour.
- Triage using evidenced based assessments, care plans which are recorded and monitored for progress to move service users through the systems.
- Sign post and advise both service users and professionals of all the commissioned serves available in Manchester.
- Support service users to participate in positive diversionary activities and seek employment.
