Job Overview

We are seeking an experienced Mental Health Practitioner to join our Community Mental Health Team. As a key member of our team, you will work with adults (aged 16 to 70) who are suffering from mental health issues, providing holistic and recovery-focused care in their own homes.

Main Responsibilities

As a Mental Health Practitioner, you will hold and manage a caseload of people and have Care Coordinating responsibilities under the Care Program Approach Guidelines. You will work with clients to identify goals as part of overall care plans, and establish and evaluate appropriate treatment programs for service users within the team.

You will also be responsible for:

  • Assessing clients and their carers' needs and accessing and signposting to community resources and provider services
  • Working with clients to identify goals and establish treatment plans
  • Participating in providing quality first-line psychosocial assessments and working proactively with multi-agencies

We are looking for a qualified Mental Health Practitioner with:

  • Professional qualification (RMN/OT/SW)
  • Professional registration
  • Experience of working in a community mental health setting
