Job Overview

We are seeking an experienced Mental Health Practitioner to join our community mental health team in Manchester. As a key member of our team, you will work with adults (aged 16 to 70) who are suffering from mental health issues, providing holistic and recovery-focused care where they are most comfortable, at home.

Main Duties

You will hold and manage a caseload of people and have Care Coordinating responsibilities under the Care Program Approach Guidelines. The team covers an ethnically and culturally diverse area, and you will have the opportunity to work with clients from a wide range of backgrounds and with a wide range of psychosocial needs.

You will work with clients to identify goals as part of overall care plans, and establish and evaluate appropriate treatment programs for service users within the team. You will also participate in providing quality first-line psychosocial assessments as part of GMMH's Functional CMHT's duty role and work proactively in forging professional working relationships with multi-agencies.

Person Specification

We are looking for a qualified Mental Health Practitioner with experience of working in a community mental health setting. You will have a professional qualification (RMN/OT/SW) and professional registration. Postgraduate qualifications in mentorship or NMP are desirable.

You will have experience of working with clients from diverse backgrounds and with a wide range of psychosocial needs. You will also have experience of using electronic recording systems and have a good understanding of the Care Program Approach Guidelines.
