Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team in Rural Warwickshire North PCN. As a key member of our Primary Care Network, you will play a vital role in ensuring the safe and effective management of medications for our patients.
Key Responsibilities- Conduct structured medication reviews for patients with complex polypharmacy, implementing prescribing decisions and requesting appropriate tests.
- Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure seamless medication management.
- Support long-term condition reviews and care home ward rounds to carry out medication reviews and de-prescribe as appropriate.
- Contribute to reductions in medicine-related hospital admissions and re-admissions by supporting patients to achieve the best outcomes from their medications.
- Identify and address medicine-related issues, acting as the interface with community and hospital colleagues.
- Manage patient and healthcare professional medicine queries, and investigate complaints involving medicine issues.
- Contribute to service improvement within enhanced services QOF and IIF, and implement drug withdrawals and alerts as necessary.
- Work with the primary care team and patients to implement NICE and other evidence-based guidelines, and provide medicine-related information and training to practice staff.
- Qualified Pharmacist with a minimum of 2 years post-qualification experience.
- Excellent interpersonal, influencing, and negotiating skills, with the ability to communicate complex and sensitive information to a variety of audiences.
- Desirable: Independent Prescriber or working towards it, with knowledge acquired through postgraduate diploma or equivalent training, and experience of working in GP practice or Primary Care Network.
