Nurse Case Manager

We are seeking an experienced Paediatric Nurse to fill a challenging and rewarding role as a Paediatric Case Manager. In this role, you will be responsible for delivering complex care packages to children in Buckinghamshire, Oxford, and West Berkshire ICB.
You will work closely with a team of carers, providing training and support to ensure they have the necessary skills and knowledge to deliver high-quality care. The successful candidate will have a minimum of 2 years' experience in the UK as an RSCN with an active NMC PIN.
- Delivering tailored care plans that meet the unique needs of each client
- Providing ongoing monitoring and support to ensure the highest level of care is delivered
- Collaborating with community teams, CHC nurses, and CCG commissioners to develop and maintain effective care packages
A supportive management style that can bring out the best in others is essential for this role. You will also need to have up-to-date experience of tracheostomy care and ventilators. A full UK driving licence is required.
