The successful candidate will play a key part in facilitating safe and supported transitions for patients in the hospital who may be threatened with homelessness or are homeless.
As a Hospital Discharge Housing Coordinator, you will work closely with healthcare professionals, social workers, housing providers, and patients to ensure seamless transitions from hospital settings to stable housing.
You will conduct homelessness assessments and the housing needs of applicants supporting them to develop a personalised housing plan and issue statutory notification letters regarding individuals' housing needs.
The ideal candidate will have knowledge of the Housing Act, housing options, support services, and local housing policies, particularly related to homelessness.
Prior experience in housing support, social care, or healthcare coordination, with a specific focus on homelessness prevention and intervention is essential.
The estimated salary for this role is £35,000-£45,000 per annum, depending on qualifications and experience.
This is an exciting time to be part of the Housing Options Service and the Council's major new Homelessness Transformation Programme.
