Job Overview

This role involves assessing and implementing Specialist Palliative Care and Advice, managing patient caseloads, and acting as a link between primary, secondary, and tertiary care. The successful candidate will also be responsible for delivering specialist training and education programs to staff and students.

The team provides a seamless service for patients, working closely with partners in primary care and the local authority. As part of this integrated provider of acute, intermediate, and community healthcare services, you will have access to opportunities for professional development and growth.

Key Responsibilities
  • Assess and implement Specialist Palliative Care and Advice
  • Manage patient caseloads and act as a link between primary, secondary, and tertiary care
  • Deliver specialist training and education programs to staff and students
Requirements
  • Registered Nurse General with Diploma in Nursing or Degree
  • Experience in end-of-life care
  • Advanced communication skills
  • V300 qualification
