Job Title: Mental Health Support Worker

We are seeking a dedicated and compassionate Mental Health Support Worker to join our team at Gray Healthcare. As a Mental Health Support Worker, you will play a vital role in supporting an individual with complex needs, including Organic Personality Disorder and dysarthria resulting from brain injury.

About the Role:

As a Mental Health Support Worker, you will be responsible for providing 2:1 support to an individual living independently in their own home. You will work closely with the individual to promote their independence, dignity, and well-being, while also ensuring their safety and security.

Key Responsibilities:

  • Provide person-centered support to the individual, promoting their independence and dignity.
  • Assist the individual with daily living tasks, such as personal care and medication management.
  • Support the individual in accessing community resources and activities.
  • Collaborate with the individual's healthcare team to ensure their needs are met.
  • Implement risk assessments and care plans to ensure the individual's safety and well-being.

Requirements:

  • Full UK driving license (essential).
  • Social care experience, ideally within mental health and with experience of supporting individuals with Schizophrenia.
  • Experience providing encouragement to individuals with complex health needs and Learning Disabilities.
  • Experience supporting individuals with challenging behaviors.
  • Experience implementing detailed care plans and risk assessments.
  • Current Right to Work in the UK and at least 6 months of employment in the UK.
